With our weekly installment program medical cannabis can be affordable to everyone.  We recognize that cost can be a barrier to the treatment patients need.  Now for just $10 per week you can be certified and maintain active orders in the Florida registry.  No large up-front cost.

A new certification includes evaluation from one of our providers with a review of medical records and recommendation and orders for use of medical cannabis.  Please note that if you have a certification from another state you will need to restart the certification process for the State of Florida.

This is the same certification as listed as above.  Because we are well acquainted with the histories of our primary care patients we can charge less for a review of records.  Not a patient?  You can book an appointment on our primary care site:  www.evolutionprimary.com

Patients who have already been certified must be seen every 7 months for a renewal of medical cannabis orders.

Patients who have been certified by a provider outside of our practice will need to be seen and evaluated prior to orders being renewed.
